IT Band Syndrome From Trail Running: Why Downhills Can Trigger Knee Pain

Let's hope your IT Band symptoms aren't as dramatic as this, requiring medical evacuation:
This runner, newer to ultrarunning, starting getting warning signals from a cranky right knee at mile 10 of a 21 mile of a trail race.
Not thinking much of it, they soldiered on until, "(the pain) went down as my shin into my foot. I kept going until the pain became unbearable. It was like someone was sticking a knife in the outside of my knee, twisting it and causing pain just radiating down my whole lower leg."
It was at mile 18 that they asked for the medics to come pick them up. Smart move.
Reality: It Band symptoms and pain can be debilitating.
And extremely common in newer trail runners, ultramarathoners increasing their weekly mileage, and even regular road runners who sign up for the occasional trail race.
(It can be especially confusing for runners who may have been told they need to run on softer surfaces because of their knees of other joints. Consequently, they hit the trails. And begin to develop other problems.)
So yes, obviously I have some exercises for you to try out later in the blog, but I want you to be aware of a few things first...

Pain on the Outside of Your Knee? A Runner’s Guide to Recovery
If I was your physical therapist seeing you for outside knee pain and getting you ready for your next 50k or 50miler, here's part of my mental "do you have ITBand syndrome" checklist:
1. is your pain pretty much localized to the outside of your knee, maybe in line with your knee cap or even above it?
2. Are you a newer runner in general? Or new to trail running?
3. Male vs female? (sorry, dudes. Men are up to 80% more likely to develop ITB compared to females)
4. Have you recently increased mileage/volume and/or speedwork?
5. Have you been adding in more down hills, specifically on more single track trails?
6. Do you have pain walking down stairs?
If you're answering YES to at least 4 of these, I'd then go on to do more specific testing...but yea, IT Band is looking likely.
IT Band Facts You Need to Know
Important facts you won't find commonly on the internet:
I need you to start thinking of your IT band as a tendon that you need to strengthen rather than stretch.
A lot of on-line treatments focus on passive recovery with a lot of rest.
And while I never want you gritting your teeth through exercises because of pain, complete rest is actually contraindicated, meaning it's NOT good for IT Band recovery.
Long story short:
We don't want you losing your aerobic, or running fitness, but more importantly, we don't you losing the tissue tolerance or general tissue strength you've built up from running (think of this as your ability to handle run time on you feet without being wiped out the next day from soreness).
So up hill treadmill walking, cycling, even gently uphill treadmill running are very viable cross training options.
And lastly, is not physically possible to stretch your ITB.
And foam rolling does not actually help heal the injury.
However, ff these feel good and help you manage pain, feel free to continue, but cautiously, making sure symptoms don't actually rebound and get worse afterwards.
Because like we mentioned above, for you to get back to the trails, we have to strengthen this giant tendon that is responsible for energy storage and release, especially during downhills (regardless of how technical they are).
You'll know you can begin returning to trail running once you can go down stairs without ANY pain and can complete exercises and plyometrics similar to this one.
As much as I would like to lay out an entire "here's how you know you blast it down hill again", this genuinely requires 1-on-1 work with either myself or another running physical therapist because each runner is going to adapt differently.
But I do want you to be aware:
Start with shorter down hills and do NOT increase both mileage + downhills at the same time if able.
Think of increasing mileage as volume and throwing in downhills as intensity.
When you increase BOTH of these, you're changing 2 variables at the same time, adding load to your body from 2 different directions.
Be kind to your body while it's healing and only change 1 variable at a time. (same goes for any kind of speed.)
You may also want to stick to wider trail paths during your recovery process as quick return to single track could make your IT Band spicy again, very quickly.
Enough chatting; let's hop into those exercises.
IT Band Syndrome Recovery for Runners: Exercises, Cross-Training & Return to Trails
Circuit:

Modified Bulgarian Split Squat
a) painful leg UP on box behind you // 3x 12 body weight
b) painful leg DOWN in front // 4 x 6-10 reps @ RPE 6-8

3 secs to descend, 3 secs to ascend

How to Get Back to Trails Without Flare-Ups
Modified Bulgarian Split Squat
Pay attention to where the ouchie leg is!
Yes, we start with the ouchie leg behind you, up on the box. This is so we can wind up that IT Band tendon initially, kind of like wrapping a ball of yarn, placing maximally tension on it, but with ONLY body weight. I want you to focus on pushing through the top of your foot during this exercise on both the descent and the ascent.
PRO TIP: you can move onto the second version once you can go down stairs without any pain, now placing the ouchie leg in front of you, making it the working limb. From here, you will progressively increase the weight as your reps get LOWER.

Lunge with Foot Elevated on Step
You can do regular lunges or....we can continue to specifically target your ITB.
THE REASON: Your ITB is directly connected to your TFL and other anterior (or forward structures) which are responsible for hip flexion (your leg traveling forward). Plot twist: it's also connected to posterior (or "behind") muscles like your glute min and med, which are more responsible for hip extension (your leg traveling backwards).
By placing your foot ON STEP of a step and performing a lunge here, we're again winding up that tendon, placing it on more stretch, AND adding a heavy weight for it handle.
Pay attention to the instructions for 3 secs descent // 3 secs descent. This is to directly target both concentric and eccentric control of both these anterior and posterior structures. When we work these muscle groups and IT Band in this way, we help prepare your ITB to handle the eccentric demands of running downhill on trails.

Always: MORE CORE!
PRO TIP: While core strength isn't directly related to ITBand recovery, I would 100% still add this to your physical therapy exercises if we were working together. I like how this exercise again, winds up that ITB (a little), forces weight onto the ouchie leg (this position should be relatively symptom free), and then asks you to create momentum with a weight and control it's deceleration. Because going down hill is a lot of controling deceleration.
I'm not saying this exercise is "like running down hill"; I realize that. But it's a great entry-point into exploring "is your body ready for similar demands?"
Plus, core. Who doesn't like fun core work?
Lateral Skater Jump
These are non-negotiable. You must do plyometrics if you want a healthy ITBand
THE KEY: Tendons store energy. Therefore, tendons need to be "stiff", not in a "it feels tight way" but in a strength-way. In "tendon-land", we don't say a tendon is strong; we say it's stiff. And plyometrics directly improve tendon stiffness and train it to store and release energy.
This side-to-side jump is a perfect example of this and does replicate some of the more techincal running you may need to do on the trail depending on what obstacles you enounter. NOT pictured here, but you can add jumping OFF of a step and jumping back onto the step as a progression when you feel ready.

Remember: getting back to running downhill requires eccentric strength and ability to control deceleration
WHY YOU NEED THIS: This is a very easy, gentle way to move into single leg deceleration work because I see a lot of runners immediatly add in some forms of jumping off of tall box. And while depth drops like these ARE GOOD! sometimes they can be a bit much and set runners back instead of helping them progress.
So I like this mash-up of sitting on a box or bench, jumping up with BOTH legs as high as you feel comfortable, and landing on 1 leg. Be sure to land quietly and with a bent knee. And realize, worse case scenario, you just sit down. The bench or box will catch you.
You start with tiny jumps with gentle landings and progress to more explosive jumps and controlled single leg landings.
WRAPPING UP
IT Band pain doesn't mean you have to give up running trails.
But getting back to your normal mileage isn't just about waiting until your knee stops hurting.
What these exercises do is help your IT Band be able to handle load again.
That means progressively rebuilding strength, tendon capacity, and your ability to control deceleration on one leg.
It also means paying attention to the things that may have increased your load in the first place: more mileage, more speed, more hills, and especially more downhill running.
So remember:
You don't need complete rest to recover. Keep your aerobic fitness with symptom-friendly cross-training when needed.
Stretching and foam rolling aren't going to strengthen the IT Band. If they feel good and help manage your symptoms, great, but they're not the main event.
Strength comes first. Progressively loading the muscles and tendon around your hip and knee is what helps rebuild your ability to tolerate running.
Plyometrics matter. Your IT Band is part of a system that stores and releases energy, so eventually you need to train it for the faster, springier demands of running.
Downhills are a separate variable. Don't increase your mileage and suddenly add a bunch of steep or technical descents at the same time.
Earn your way back to the trails. Being able to walk down stairs pain-free is one useful checkpoint before progressing toward running, jumping, and eventually longer or more technical descents.
And if you're a trail runner or ultrarunner, remember that your goal isn't simply to make your knee feel "good enough" to run again.
Your final goal is to build a body that can handle all the demands you throw at it.
Because the trail will still be there when you're ready for it.
